Awakened to Freedom Appointments

By checking "I Agree," I acknowledge that team members from Awakened to Freedom Ministry have voluntarily agreed to minister to me. I understand that this session is not a professional counseling meeting and that none of the team members are licensed counselors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.
I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the help that I might receive from the team members of this ministry.
Awakened to Freedom Ministry team members offer biblically-based spiritual services to those who desire them. I understand that, if I receive ministry from Awakened to Freedom Ministry, the team is committed to respecting the disclosed information, and these are confidential sessions unless the information disclosed would alert team members to potential harm to me, the client, or to others. Awakened to Freedom Ministry observes mandated reporting practices and is a mandated reporter, and therefore is obligated by law to report abuse.
I agree to hold Awakened to Freedom Ministry, Conejo Valley Healing Rooms Inc., and its team members harmless from any and all liability, loss, or damage of any kind that may arise as a result of assistance that I have received, or from my involvement with, Awakened to Freedom Ministry.
